New Patient Process

  • Step 1 - Make An Appointment

    Becoming a Thrive Family Medicine patient is simple and easy. 

    Call to schedule an appointment with one of our providers, (864) 412-2777.

  • Step 2 - Fill Out Paperwork

    To help make your first appointment check-in even easier, use the button below to fill out and print your new patient forms ahead of time. 

  • Step 3 - Arrive Early to Appointment

    Plan to arrive 30 minutes prior to your first new patient appointment. This applies to every new patient - even those who have already filled out the patient forms ahead of time. There are forms that we are unable to provide ahead of time due to privacy regulations so they must be filled out in the office before your appointment. This also gives your provider time to review your new paperwork before seeing you in the exam room. 

    Don’t forget to bring your printed and completed forms if you have them, as well as your insurance information.

  • Step 4

    See your new provider and breathe a sigh of relief. Please let our front desk staff know during check-out if you have additional questions or concerns about your visit.

General Office Policies

After Hours Emergencies

If you have a medical emergency, call 911.

Our providers are available 24 hours a day, 7 days a week for advice about your medical problems. Our after-hours call service can be used for urgent or acute problems, but routine medication refills will not be handled after hours.

To reach our providers after hours, simply call the main office number 864-412-2777. Your call will be routed to the on-call provider who will return your call within 30 minutes.  If you have not heard back from a provider in this time frame, please call again noting that you have not received a response so that we can be sure that we did not miss your call.

Payment Policy

Patients are expected to pay for co-pays, deductibles, and co-insurances at the time of service. Any overdue accounts that go beyond 60 days will be turned to an outside collections agency, and agency fees will become the patient’s responsibility.

Medication Refills

Medications will be refilled during office hours only. Refills requested outside an office visit may incur a $10 fee. Medications will not be refilled by paging the on-call provider after hours.

Holidays & Weather Closings

Our office will be closed on the following holidays: New Years Day, MLK Day, Memorial Day, July 4th, Labor Day, Thanksgiving day,  Christmas Day.

In the case of inclement weather, we will try to contact you directly or via the portal about your appointment. If we are not able to do so, please check our website, Facebook page, or local news station for list of closings.
